can choose to see a doctor not on the preferred list, but you will
have to compensate part of the bill and have to pay a deductible.
Some PPOs need claim forms.
Point of Service (POS): This plan gives
you two different options each and every time you use health care
services: The first option is to apply the plan just like a HMO and
the second option is to apply your plan just like an indemnity plan.
While you are using your health care plans like indemnity insurance,
you are responsible for a deductible and a percentage of your
receipt.
